Hazel Dorine Carlson Nee Ennis Passed away Friday December 5, 2014 at Holy Cross Hospital In Chicago, Illinois. Hazel is survived by her daughters Hazel D Carlson, Elaine L. Carlson, numerous grandchildren, great-grandchildren, and nieces and nephews, as well as a brother Thomas Ennis. She was preceded in death by her husband Richard A Carlson, a son Henry E. Carlson and a brother Ben Ennis. A funeral service will be held at the Woodlawn Memorial Park I Cemetery Thursday December 11, 2014 at 1 PM.
